INSURANCE AND FEES

Fee For Service Rates:  $150 for 45/50 minute session and $275 for 90-minute session.

NO INSURANCE – NO PROBLEM. I ACCEPT ALL SELF-PAY CLIENTS AND CAN PROVIDE A SUPER BILL RECEIPT.  

 

INSURANCE INFORMATION:If your insurance is not listed below,I am considered out of network: If I am considered an out of network provider,  I will provide you with all the paperwork for you to fill out of network. Generally, if you have out of network benefits, they will reimburse you at least 50% or more, and they pay you directly. Please call your insurance provider for more information to submit claims.

 

No worries, as I’ll provide you with all the paperwork that you’ll need for easy submission of claims. I accept Health Savings or Flexible Spending Accounts. (All credit cards are charged a minimal surcharge.)

 

I am in network with the following companies:

• UNITED HEALTHCARE/OPTUM/UMR

• CIGNA

• HUMANA

You will need to contact your insurance provider to find out your specific coverage for Mental health benefits. Payment for services is required at appointment time.

Payments I accept: Venmo, Zelle, Paypal, Square payment, Apple Pay, cash or check. All returned checks will be charged a $35 fee and will accept only cash payment for future appointments.There is a 24-hour STRICT cancellation policy/no show fee of $85, which insurance will not reimburse, so please be aware of this policy. (I must adhere to the strict cancellation policy as I have many clients on WAITLIST)
